2. From the overview, click on 'Add new' and select 'Question and answer'

3. Assign each new question a category

4. Save and publish

You can always edit your questions and answers, change their order and choose other categories.

Yes. Follow these steps to add media:

1. Go into the app's settings

2. Click on Manage Questions

3. Create or select the question you want to add media to

4. When editing your answer, click the video, image or gif icon

5. Add media from your library and save

You can change the header from the Settings tab in the app. If you don't want to show the header, just disable it under 'Info to show'.
